Doctor on the Edge Episode 4 Recap & Review: Ji Ui Faces His Worst Fear

Doctor on the Edge Episode 4 Recap – Episode 4 of Doctor on the Edge opens with Ji Ui rushing toward the health center after spending time outside. Before he arrives, Jucheon spots him but decides not to bring up the gossip that’s been spreading around the village just yet.

The following day, Ji Ui is dealing with the consequences of that night and comes down with a cold. While working in the consultation room, Ha-ri shows him more photos, and the two share a lighthearted moment together. Chiyeon notices them laughing, though the mood soon changes when Ha-ri receives an urgent call from her grandmother.

Her grandmother has suffered a dizzy spell in the kitchen and injured her head. When Ha-ri arrives, she discovers medication that her grandmother admits she has stopped taking. Ji Ui encourages her to continue the treatment and visit the health center, but the older woman refuses because she doesn’t want to burden Ha-ri with more worries.

During the visit, Ji Ui reveals something surprising. He says he noticed an anti-cancer medication the very first time he met her grandmother. Rather than judging her decision, he comforts her and reassures her that her feelings about the situation are completely understandable. It’s a quiet scene, but one that really stood out to me because of how gently Ji Ui handled it.

Meanwhile, back at the health center, Nurse Eom notices that Jucheon seems distracted. The reason becomes clear once Ji Ui and Ha-ri arrive.

Jucheon pulls Ji Ui aside and finally shares the rumor that’s been circulating. According to village gossip, a nurse has supposedly charmed three different doctors and earned herself the nickname of a “gold digger.” Jucheon believes the nurse in question is Ha-ri and claims that her next target is Chiyeon.

Ji Ui is caught off guard. For a moment, he actually thought Jucheon was talking about him.

A brief flashback reveals villagers gossiping about Ha-ri and Chiyeon, fueling the rumors even further. Ji Ui’s doubts grow when he later stops by to deliver forgotten health education materials and sees Ha-ri and Chiyeon joking around together during a CPR demonstration.

At the same time, Ha-ri and Chiyeon discuss the rumors themselves. Ha-ri is clearly uncomfortable with all the attention and asks him not to do anything that might encourage more speculation.

That evening, Ha-ri runs into Ji Ui at a convenience store. She tries to talk to him, but he’s noticeably colder than before. The gossip has already affected how he sees the situation. When he repeats the hurtful labels people have been using, including “doctor hunter” and “gold digger,” Ha-ri is visibly wounded by his words.

I honestly felt bad for her during this scene. She spends the entire episode dealing with assumptions made by other people, and this moment really drives that home.

Elsewhere, Jucheon attempts to warn Chiyeon about the rumors surrounding Ha-ri. Unfortunately, Chiyeon has earphones in and misses every single word. The timing couldn’t have been worse.

Later, Nurse Eom calls Jucheon regarding their accidentally swapped phones and asks him to bring hers back. When he arrives at her house, he notices a table filled with food and realizes she’s spending her birthday alone.

Instead of leaving immediately, he sits down and shares dinner with her. Nurse Eom seems genuinely surprised by the gesture. During their conversation, Jucheon reveals that birthdays were never happy occasions for him because he was often bullied growing up.

It’s a small subplot, but it adds another layer to his character.

Back at the residence, Ji Ui can’t stop thinking about his argument with Ha-ri. Before he can sort through his feelings, an emergency call comes in.

A man has seriously injured his tongue after getting caught on a fishhook. The team quickly responds, and Chiyeon provides a helpful suggestion during the treatment.

The next morning brings another awkward situation. Jucheon wakes up at Nurse Eom’s house with very little clothing on and immediately apologizes. Nurse Eom tells him to leave quietly before anyone notices and starts another round of rumors. By the time he reaches the health center, he’s already twenty minutes late.

The staff barely have time to process that before receiving news of an important visitor.

Choi Hyang-mi, the Director of Public Health Care Services, is arriving to deliver an official warning. The announcement immediately puts everyone on edge since repeated warnings can lead to penalties for civil servants, including pay reductions, restrictions on leave, and even extensions of mandatory service.

Nurse Hwang instantly switches into preparation mode, ordering everyone to clean up and make sure they’re wearing their identification badges. Since Ji Ui still hasn’t received his official ID, he’s handed an old one that once belonged to Kim Du-pal.

During all this chaos, a young patient arrives for treatment. Jucheon attempts acupuncture, but the child starts crying. What happened next made me laugh more than I expected, Jucheon starts crying right along with her.

When Director Choi finally arrives, she delivers the governor’s message and scolds Ji Ui for failing to remember the governor’s name. Thankfully, she quickly reveals that she doesn’t personally agree with the harsh treatment and eases the tension.

Not long afterward, the young girl from Jucheon’s clinic comes running out. Director Choi prevents her from falling but injures herself on broken glass in the process. Ji Ui treats her wounds, further demonstrating his medical skill under pressure.

Soon after, the village chief announces that ferry and ship services may be suspended due to dangerous weather conditions.

The director later treats the staff to dinner. During the gathering, Nurse Eom brings up the subject of one-night stands while talking to Ha-ri. Unfortunately, Ha-ri assumes the conversation is connected to the rumors involving her and Chiyeon.

Ji Ui follows her outside to finally address the tension between them. However, Ha-ri remains upset. She explains that she worked with many male doctors while living on the mainland and naturally ended up dating some of them. That doesn’t make the rumors true, nor does it justify the labels people have attached to her.

Before they can fully resolve things, another emergency interrupts them.

The young girl treated earlier is suddenly having difficulty breathing.

An emergency medical boat is arranged immediately. Ji Ui prepares for an intubation procedure while Director Choi helps coordinate the response. She asks him to accompany the patient, but he hesitates.

The reason quickly becomes obvious.

Ji Ui has very little experience handling pediatric cases, and being on the boat forces him to confront his fear of the sea.

As rough waves crash around them, memories from his past begin resurfacing. He recalls a traumatic incident involving Hwayeong and Seonu while they were trying to save a patient on a boat.

Ha-ri stays by his side and helps him focus on the task at hand. Guided by Director Choi, Ji Ui starts the intubation procedure despite the panic building inside him.

The sequence is incredibly tense. Every flashback threatens to pull him away from the present moment, yet he keeps going.

Thankfully, the intubation succeeds.

For a brief moment, it seems like the crisis has passed.

Then disaster strikes.

As the boat continues through the stormy waters, Ji Ui suddenly falls overboard.

The episode ends right there, throwing everyone into panic and leaving viewers with a massive cliffhanger.

Episode 4 Review

Doctor on the Edge Episode 4 did a great job expanding Ji Ui’s backstory without revealing everything at once. We finally get a clearer picture of why he reacts so strongly to boats and rough seas, and those flashbacks add a lot of weight to the final sequence.

I also liked Director Choi’s introduction. The episode initially presents her as someone arriving to hand out punishment, but she quickly proves herself to be practical, supportive, and genuinely invested in public healthcare. I’m curious to see what role she’ll play moving forward.

Ha-ri’s storyline was another highlight. Watching rumors spread through the village showed how quickly assumptions can take on a life of their own. The accusations against her felt unfair from the start, and this episode makes that even more apparent.

The emergency boat sequence easily steals the show, though. Between the dangerous weather, Ji Ui’s trauma, the child’s condition, and the successful intubation, the tension kept building until that final shocking moment.

My jaw dropped when Ji Ui went into the water. It’s the kind of ending that makes waiting for the next episode feel much longer than a week.
