Doctor on the Edge Episode 3 Recap – Episode 3 opens with yet another rough day for Ji Ui. The village chief storms into the ferry terminal and publicly humiliates him, dumping trash in front of him and calling him a liar. The chief explains that the insurance company ruled his condition as angina rather than a myocardial infarction, meaning his 15 million KRW hospital bill won’t be covered.

Ji Ui refuses to back down. He explains that angina can easily progress into a heart attack and insists the chief could have died if he had followed the original suggestion to take antacids instead. The medical explanation doesn’t help the situation. The chief remains furious.

On the way back, Ha-ri and Ji Ui stop by the Cliff Side Restaurant, where Miss Hwang warns him that the village chief never lets grudges go. She turns out to be right. The chief has already instructed island residents not to welcome the health center staff, especially Ji Ui.

Watching Ji Ui get frozen out by almost everyone on the island is frustrating. Even when Ha-ri tries to comfort him, it feels like he’s carrying the weight of the entire conflict alone.

The chief’s retaliation doesn’t stop there. He recruits health center workers to clean trash around the ferry terminal, while Ji Ui continues seeing patients. One of them is Jang-su, who immediately challenges him about prescribing only three days of medication.

Ji Ui recommends additional tests and X-rays on the mainland, but Jang-su strongly rejects the idea. The conversation quickly turns tense. After repeated resistance, Ji Ui finally loses patience and asks him to wait outside.

Meanwhile, Jucheon and Ji Ui receive treatment from Chiyeon. Jucheon vents about everything that has happened and even complains about having to purchase medical supplies himself. Chiyeon remains practical, reminding him that he still has responsibilities as both a medical officer and a soldier. He also makes it clear that filing formal complaints won’t accomplish much.

The conversation eventually shifts to Chiyeon’s past. Jucheon reveals that Chiyeon was once sued after treating two patients suffering from fish poisoning. One patient died while another survived. Both families ended up taking legal action against him.

That revelation clearly sticks with Ji Ui. It also plants another question in his mind: if things go wrong, will Ha-ri stand with him or with the islanders?

Over the next few days, Ji Ui starts giving residents exactly what they ask for, including thirty-day prescriptions. It feels less like medical judgment and more like surrender.

Ha-ri notices the change immediately. When she asks if he’s okay, Ji Ui responds coldly. He says he’ll simply finish his service and leave. Nothing more.

The answer visibly upsets her.

While Ha-ri and Chiyeon share a lighthearted moment about her sunscreen habits, Ji Ui is stuck delivering parcels around the island. During his rounds, he notices a package intended for Ha-ri and decides to text her. Unfortunately, he accidentally sends a selfie along with the message.

That awkward mistake becomes one of the episode’s funniest moments.

Back at the health center, Miss Hwang assigns Ji Ui the task of organizing supplies. At the same time, Ha-ri and Chiyeon travel to Madeokdo for medical work.

Things take an unexpected turn when an elderly woman arrives carrying a dog that needs treatment. Chiyeon is completely baffled and walks away. Ha-ri follows him, only to learn from the village chief that the woman believes the dog is actually her deceased son.

Chiyeon eventually contacts a veterinarian for help. The dog receives treatment, but not before biting him.

Later, Ji Ui treats the restaurant owner’s badly cut finger after a kitchen accident. Concerned that she may need stitches, he recommends a trip to the mainland. He even exaggerates the possibility of nerve damage, thinking she’ll be more willing to go if she understands the risk.

Elsewhere, Ha-ri treats Chiyeon’s dog bite. During the conversation, Chiyeon admits he’s afraid of dogs but stepped in anyway because he knew the situation mattered to her.

Just when it seems their work is done, the elderly woman returns. This time, the dog has been injured by a splinter. Ha-ri, Chiyeon, and the veterinarian work together to remove it and treat the wound.

Because she misses the last boat back, Ha-ri is forced to stay overnight on Madeokdo.

During the evening, Chiyeon brings up the story of how he first met Ha-ri. A brief flashback shows them helping an accident victim with CPR. Afterward, he returned her phone, but Ha-ri mistakenly thought he was asking for her number. It’s a small memory, but it adds a nice bit of humor to their dynamic.

Back on the main island, Ji Ui drops off Ha-ri’s parcel at her house. That’s when he accidentally overhears a conversation between Ha-ri and her grandmother.

Trying to leave unnoticed, he trips and injures himself.

The grandmother catches him immediately and invites him inside to treat the wound. While there, Ji Ui notices pain medication and asks about her health. She dismisses it as simple migraines and avoids discussing it further. Before he leaves, she saves his phone number in case of emergencies.

Over on Madeokdo, Ha-ri and Chiyeon prepare to sleep. Ha-ri initially assumes Chiyeon is worried she might cross personal boundaries.

Ironically, that’s exactly what happens.

While asleep, she unknowingly throws a leg over him, creating another surprisingly funny moment.

The following day, Ji Ui arrives at the health center looking for Ha-ri. Chiyeon has returned, but Ha-ri is still away.

At the same time, a man named Ko Changmok calls to check whether there are any problems. He is later shown holding a press conference about upgraded medical facilities on the island. Once the cameras are gone, however, his tone changes completely. He scolds the director for weak oversight and instructs her to punish Ji Ui’s medical center.

Meanwhile, Jang-su returns with complaints about worsening cough symptoms and asks for stronger medication.

This storyline ended up being one of the most important parts of the episode.

Miss Hwang notices changes in Jang-su’s medication history while reviewing his records. She reveals that he previously suffered a heart attack and required a stent procedure.

The moment Ji Ui hears this, he immediately realizes the stronger medication could be dangerous.

He rushes out of the clinic and desperately tries to reach Jang-su before it’s too late.

Jang-su remains completely unaware of the danger as he rides home. Eventually, Ji Ui catches up with him, confronts him about hiding his medical history, and confiscates the medication.

Instead of getting angry, Jang-su thanks him for saving his life.

From there, the mood of the episode begins to shift.

On his way back, Ji Ui encounters another patient who appreciates his exercise advice. He also learns that the restaurant owner followed his recommendation and traveled to the mainland for treatment.

Then comes an even bigger surprise. The village chief arrives carrying seafood as a gift for the health center staff. After everything that happened earlier, I honestly didn’t expect this turnaround.

Ha-ri explains what really happened. The insurance company had attempted to mislead the chief regarding his claim. With the misunderstanding cleared up, the chief leaves on good terms and even says his wife will bring lunch for the staff the next day.

That evening, Ji Ui and Ha-ri run into each other at a convenience store.

Naturally, Ha-ri brings up the accidental selfie.

The teasing helps break the tension between them. As they talk, Ji Ui opens up about Jang-su’s case and admits that he constantly worries someone will file a complaint against him and jeopardize his ability to continue working on the island.

In response, Ha-ri takes him somewhere special. She shows him a field filled with fireflies.

It’s one of the quietest scenes in the episode, yet it says a lot. Watching Ji Ui stand there in amazement while Ha-ri apologizes for always telling him to simply endure everything felt surprisingly touching.

Back at the health center, Jucheon invites Chiyeon out for drinks. Chiyeon declines, blaming a sleepless night. Jucheon immediately starts imagining reasons why, but his attention is interrupted by a flood of messages appearing in a group chat.

The topic catches everyone off guard. The messages discuss a so-called “doctor hunter” connected to Ansong Hospital. Jucheon quickly jumps to a conclusion and suspects the person being discussed is Ha-ri.

Meanwhile, Ji Ui asks Ha-ri why she chose Chiyeon instead of him for the Madeokdo trip. Her answer is simple. The location wasn’t within walking distance and required an hour-long boat ride.

That’s when Ji Ui finally realizes something. She wasn’t excluding him. She was looking out for him.

Ha-ri then invites him to join her on the next visit, ending the episode on a much warmer note.

Review: A Strong Character Episode With a Surprising Final Twist

Doctor on the Edge Episode 3 spends less time on major medical emergencies and more time developing relationships, and it works surprisingly well.

The biggest strength here is Ji Ui’s growth. Earlier episodes showed him constantly clashing with the island residents, but Episode 3 begins to soften those edges. Watching him earn trust little by little feels rewarding because it doesn’t happen overnight.

The Jang-su storyline is especially effective. It reminds us why Ji Ui’s stubbornness can sometimes be a good thing. Had he simply given the patient what he wanted, the consequences could have been serious.

I also enjoyed the quieter moments between Ji Ui and Ha-ri. The accidental selfie, the fireflies, and the reveal that she had been looking out for him all add warmth without forcing romance too quickly.

Then there’s the final reveal about the mysterious “doctor hunter.” That accusation came out of nowhere. Right now, it feels hard to believe the story is as simple as the rumors suggest. Based on everything we’ve seen so far, there’s probably much more to Ha-ri’s past than people realize.

And if there’s one thing I’d like to see more of going forward, it’s the island itself. The series shines whenever it slows down and lets us spend time with the people and scenery that make this setting feel unique. The firefly sequence was a perfect example of that.
