Kim Ji-won’s Gye Su-jeong swaps surgeries, shoves a VIP donor aside and leaves a trail of clues pointing to a mysterious “Doctor X.”
Doctor X: Mafia in White Episode 2 (the SBS and Netflix medical thriller that aired October 10, 2026) is the hour where Su-jeong stops being just the strange new surgeon and becomes a real problem for everyone at Guseo University Medical Center (GUMC). Two operations get scheduled for the same time, a wealthy donor throws his weight around, and a teenage girl’s life lands in the middle of it. If you need a refresher before moving on to the next chapter, here is the full breakdown, followed by my honest take.
Recap: The VIP, the Teenager and the Surgeon Who Won’t Take Orders
A Nightmare, a Donor and a Director Who’s Had Enough
Su-jeong wakes up from a nightmare about the fire outside the White Wings Medical Corps building. Over at GUMC, Sun-ae brings Director Bu Seung-gwon up to speed on Park Gwang-cheol, a VIP donor about to go under the knife.
Director Bu is not happy. His doctors are being pushed to bend over backward simply because Gwang-cheol writes big checks, so he asks Sun-ae to dig into every VIP donor on the books. Meanwhile, the doctors talk about the workers protesting outside, who say Gwang-cheol hasn’t been paying his employees. Director Bu then asks Dr. Do Yun-sik whether he’s willing to perform the surgery, while Sung-gyu collects Gwang-cheol from his car and wheels him inside.
The Bus Ride That Changes Everything
On her way to GUMC, Su-jeong takes the bus and bumps into a teenager named Lee Ju-hui, who collapses right in front of her. Su-jeong carries the girl into the medical center and bumps Gwang-cheol out of his wheelchair so Ju-hui can get proper treatment. Poor Sung-gyu ends up carrying the VIP to his hospital room.
In the staff room, Dr. Bae Heung-gon teases Sung-gyu, and the two talk about how Gwang-cheol picked Dr. Do as his surgeon on a whim. They try to get Su-jeong’s attention, only to realize she’s wearing noise-cancelling earphones. Dr. Do asks her to pitch in with menial work like ghostwriting paperwork, but Su-jeong calmly points out that her contract covers surgeries and nothing else.
Dessert Diplomacy in the Cafeteria
Sung-gyu asks Tae-kyeong to keep an eye on Su-jeong, so Tae-kyeong follows her to the cafeteria and offers to treat her. She orders several desserts. While he tries to make small talk, she tells him she has a lot of debt and that Director Jang Hee-sook gives her a monthly allowance of 100,000 won.
Tae-kyeong feels for her. He assumes Hee-sook is mistreating Su-jeong and forcing her to work for the Locum Tenens Agency, so he offers to buy even more desserts. I couldn’t help smiling at how sincerely he misreads the situation.
Gwang-cheol Throws a Tantrum
Back in the medical center, Gwang-cheol gives Director Bu and the other doctors an earful about Su-jeong’s behavior. He reminds everyone that he needs to survive his surgery and orders them to make sure he doesn’t die. As he’s being taken out, he spots a woman holding the same jacket the protesters wear, and he tells security to throw her and her wheelchair-bound sister out.
Su-jeong steps in and tries to stop the guards. Then Ju-hui walks outside and tears into Gwang-cheol for docking her aunt’s pay. She lunges at him, and security pulls her away. Ju-hui’s mother, who uses a wheelchair, tries to hold her back. In the middle of the chaos, Su-jeong wheels Gwang-cheol to the parking lot and leaves him in a parking spot. I’m not going to pretend I didn’t laugh out loud at that.
Dr. Jo Won-jin checks on Ju-hui afterward and explains her rare health condition to Tae-kyeong and Su-jeong. Later, in the cafeteria, two internal medicine doctors make a scene and accuse Su-jeong of overstepping because she diagnosed a patient as a surgeon. The surgeons and internal medicine doctors go back and forth while Su-jeong ignores them and goes for more food.
Two Surgeries, One Surgeon with Strong Opinions
That afternoon, the surgeons meet to go over upcoming operations. Dr. Do lays out his plan for Gwang-cheol, but Su-jeong objects and asks him to take a different approach. Dr. Do and Dr. Bae insist the procedure is complicated and risky, and Su-jeong simply says she can do it.
Then comes the bad news from Sung-gyu: because Gwang-cheol wants his surgery prioritized, Ju-hui’s operation gets pushed back. Director Bu announces that both surgeries will happen at the same time, with Su-jeong operating on Ju-hui and Dr. Do operating on Gwang-cheol.
Jenga Night and Hee-sook’s Long Game
That night, Su-jeong plays Jenga with Hee-sook and her friends, Yang Woong-gu and Yoo Mi-kyung. The group agrees that Dr. Do doesn’t have the surgical skill to handle Gwang-cheol’s case. Hee-sook argues that hospital hierarchies run on reputation and office politics rather than talent, and she says they need to wait for the right opening to bring GUMC down.
Ju-hui Says No, Su-jeong Says Yes
The next morning, Tae-kyeong tells Ju-hui’s mother and aunt that Su-jeong will perform the surgery, explaining that Dr. Do, who had promised to operate on Ju-hui, is otherwise occupied. Ju-hui refuses to go through with it because her chances of survival are low. She tells her mother she’s tired of being sick and doesn’t want to be a burden on her mother and aunt anymore.
This is the moment I felt the episode shift. Su-jeong reasons with her and promises to save her life, and Ju-hui finally agrees.
Later, Dr. Do talks to Su-jeong about the surgery, but she asks him to switch with her, telling him he isn’t competent enough to operate on Gwang-cheol. That night, Dr. Do replays her words in his head while Su-jeong skips dinner, her mind on the operation. The next day, right before the surgeries begin, she reminds him of his incompetence once more. Brutal.
The Surgery Day Reversal
Su-jeong operates on Ju-hui and Dr. Do operates on Gwang-cheol. Partway through, Dr. Do realizes Gwang-cheol’s cancer has spread abnormally. Instead of trying to finish, he decides to close the patient up, and Director Bu calls Su-jeong in for help.
At the same time, Ju-hui’s mother and aunt find a letter Ju-hui wrote to them and break down reading it. Tae-kyeong tries to stop Su-jeong from walking away from Ju-hui’s surgery for a bigger fee, but she ignores him. She goes to operate on Gwang-cheol while Dr. Bae takes over for Ju-hui, and Dr. Bae is stunned by how fast Su-jeong completed the hemostasis on Ju-hui’s body.
Dr. Do pushes back on Su-jeong’s proposed approach to Gwang-cheol, but Director Bu orders him to follow her instructions. She takes a valve from Gwang-cheol’s leg and places it in his pancreas. Then she notices her shift is over and asks Dr. Do to close up before she clocks out. He’s furious, but there’s nothing he can do. Hee-sook, meanwhile, reminds Director Bu about the extra payment owed for Su-jeong’s help with both operations.
Fallout and the “Doctor X” Clue
Director Bu later confronts Dr. Do for trying to close Gwang-cheol without finishing the surgery. Dr. Do breaks down in tears and insists he didn’t abandon his patient. In the locker room, Dr. Bae says he suspects Su-jeong planned to handle both surgeries herself. Tae-kyeong goes looking for her and finds her checking on Ju-hui, who is resting with her mother at the bedside. Gwang-cheol’s family, for their part, is upset that he survived.
The next day, Hee-sook takes payment for Gwang-cheol’s surgery in cash and says he needs to stay alive so he can pay for his crimes. Then Dr. Bae and Sung-gyu discuss how Su-jeong nearly put a patient at risk with a dangerous technique. Dr. Bae researches it and finds that the only other patient operated on this way died two months after surgery. That patient was treated at White Wings Medical Corps, and the doctor was listed as Doctor X. The episode ends with Director Bu confronting Su-jeong over her cockiness about never failing.
Review: Why Su-jeong Is So Much Fun to Watch
This was another strong hour, and a big reason is how little Su-jeong cares about the gossip and politics swirling around her. She’s so unbothered by the way other doctors treat her that she doesn’t even register it as bullying until Tae-kyeong points it out. Tae-kyeong has quickly become one of my favorite parts of the show, and I really enjoy his scenes with her.
The writing keeps layering on Su-jeong’s quirks. She rushes to eat lunch at exactly 12 p.m., and she walks out at 6 p.m. even when a surgery is still underway. She isn’t at GUMC to win anyone over or carve out a place for herself. She’s a confident surgeon who doesn’t worry about what people think, and watching her put the arrogant doctors around her in their place is a real treat.
What Works
- Su-jeong’s contract-first attitude. Refusing paperwork, protecting her lunch hour and clocking out mid-drama makes her feel like a specific person, not a generic genius.
- The Ju-hui storyline. Her refusal, the letter to her family and Su-jeong’s promise give the surgery actual emotional stakes.
- Dark humor. The parking-spot moment and Tae-kyeong’s dessert offers keep the tone from getting too heavy.
- The Doctor X clue. Ending on the White Wings link gives the weekly cases a larger mystery to hang on.
- Hee-sook’s quiet scheming. A Jenga game with a plot to bring down a hospital is a great pairing.
What Doesn’t
- Gwang-cheol is a cartoon villain. An entitled VIP who orders staff around is a familiar type, and he gets very little nuance here.
- Dr. Do is being set up as a punching bag. Su-jeong keeps reminding him of his incompetence, and I wonder how much more the show can get out of that dynamic before it feels repetitive.
- The quirk list is close to maxed out. The lunch and clock-out gags landed well, but another round of them could start to feel like a formula.
- Hee-sook’s endgame is still vague. We know she wants to bring GUMC down, but not how or why, and that keeps a few scenes feeling like setup.
Final Score
8.5/10. Su-jeong is a joy, the Ju-hui subplot hits hard, and the ending hooks you for next time. A few one-note supporting characters keep it from a perfect score.
