Doctor X: Mafia in White Episode 10 Recap & Review: The Surgery Succeeds, but the Real Fight Is Only Starting

Episode 10 of Doctor X: Mafia in White (aired November 7, 2026 on SBS, streaming on Netflix) delivers the payoff to Hyeon-a’s operation, then yanks the rug out with a final scene that changes how we see Tae-kyeong, Won-jin and a mysterious patient from Su-jeong’s past.

Episode 10 of Doctor X: Mafia in White picks up right where that nail-biting cliffhanger left off, with Su-jeong in the OR, Hyeon-a’s life on the line and, for the first time, a roomful of GUMC doctors who actually show up. Kim Ji-won’s surgeon finally gets her team, the Magnolia Society finally gets its comeuppance in the form of a lawsuit threat, and then the closing minutes throw in a twist about a patient named Seung-hyo that I did not see coming. There’s a lot to get through, so let’s start with the full recap, then I’ll tell you what I loved, what bugged me and how I’d score it.

Doctor X: Mafia in White Episode 10 Recap: Rebellion in the OR, a Lawsuit Threat, and a Shocking Epilogue

Hee-sook Remembers Lagota, Then Scrubs In

The episode opens with Hee-sook thinking back to the advice she gave Su-jeong in Lagota: always stand by your patients. Her memory lands on a night when she rushed in carrying a heart, injured but determined, to help Su-jeong with a heart transplant. Back in the present, she walks into the OR to help with Hyeon-a’s surgery, joining Dr Do, Tae-kyeong and Won-jin.

Director Bu Breaks Ranks in the Auditorium

Upstairs, Director Bu refuses to cast his vote for the strike. Instead, he tells the assembled medical staff about Hyeon-a and explains that nobody at GUMC has the skill to perform the surgery she needs to survive. He calls helping a patient like her his moral duty and heads for the OR, with Dr Bae behind him. Dr Bae asks Seong-gyu to come along, but Seong-gyu won’t leave the auditorium. Meanwhile, the Magnolia team hurries to the OR to see what the doctors are up to.

Chairman Kim, Mr Gu and Mayor Im Hit a Wall

Chairman Kim, Mr Gu and Mayor Im are furious that the senior doctors have gone against the strike. Director Bu fires right back, accusing them of trying to stall the surgery, and uses his authority as director to cut the microphone connection while Seong-gyu watches from outside. Chairman Kim floats the idea of sending security into the OR. Seong-gyu shoots that down: contaminating the room while a child’s organs are lying exposed would badly damage the Magnolia Society if the story got out. He also reminds them about the CCTV cameras in the operating room, so they decide to hold a meeting about the consequences of Director Bu’s actions. I’ll admit it, watching Seong-gyu use their own fear of bad press against them was a nice surprise.

Organs Back In, Final Stitch, and a Hand on Hyeon-a’s Heart

Seong-jin arrives in the OR to help just as the doctors begin placing Hyeon-a’s organs back inside her after removing the cancerous growth. Everyone watches without breathing, waiting for her organs to turn pink, the sign the surgery has worked. They do. Su-jeong puts in the final stitches and the surgery is a success, and the room breaks, especially Seong-jin, who’s in tears.

Su-jeong thanks everyone for showing up, which leaves Director Bu, Dr Do and Dr Bae visibly embarrassed. They ask her to do what she always does after a surgery and rest her hand on the patient’s heart. She takes Hee-sook’s hand and guides it so that both of them are touching Hyeon-a. This scene completely got me.

The Team Dinner and the Bus Stop Talk

The doctors tell Hyeon-a’s mother the surgery went well. Ga-eul and her sister thank Su-jeong for saving their friend. Hee-sook then invites the doctors who helped to a team dinner. Tae-kyeong tries to give a toast, but Su-jeong just starts eating. Dr Do and Dr Bae are stiff at first, then relax once Director Bu compliments Su-jeong. Tae-kyeong looks stunned as the table dissolves into chaos and the doctors actually bond.

Mi-gyeong and Eung-gu wonder if Hee-sook is happy that Su-jeong has made friends at GUMC. That night, Su-jeong is dozing as Hee-sook, Mi-gyeong, Eung-gu and Tae-kyeong walk her to the bus stop. There, Hee-sook tells Tae-kyeong that Su-jeong is her best friend’s daughter, and that the four of them live together though they aren’t related. Tae-kyeong shares that he isn’t biologically related to his own family either, because he was adopted. They talk about having a family of your own. When Tae-kyeong gets on his bus and finds messages from his parents, he writes back with a sense of pride after helping with Hyeon-a’s surgery.

Guilt, Drinks and a Text Seong-gyu Can’t Finish

Dr Do and Dr Bae have drinks after dinner and talk about Director Bu’s vision loss while remembering how it felt to operate on Hyeon-a as a team. At home, Seong-gyu checks on his niece and thinks about what Dr Bae said. He feels guilty for staying out of the surgery and tries to text Dr Bae, but he’s still deciding what to write when Dr Bae sends him an update on Hyeon-a’s condition. It breaks him, and he cries from the guilt.

The Hammer Falls: Suspensions and a Terminated Contract

The next morning, Seong-jin and Won-jin wait for word from the Magnolia Society, knowing they’ve rebelled. Dr Do, Tae-kyeong and Dr Bae read the circular announcing suspensions and disciplinary action against every doctor who helped Su-jeong. Her freelance contract with GUMC is terminated.

Sun-ae asks Director Bu to talk to the Magnolia Society and ask them to reconsider his suspension, and she lists the other committee members. Dr Do and Director Bu realize they have no way out. Over a meal, Su-jeong, Tae-kyeong, Won-jin and Seong-jin talk through the suspension. Won-jin warns that Hyeon-a’s family could sue the doctors if the IRB review of the surgery finds them guilty.

A Building Fire and Seong-jin’s Choice

Seong-jin gets a call about a building fire and rushes to the ER as patients arrive. Won-jin reminds him of the disciplinary action, but he refuses to turn the victims away. Su-jeong hurries into the hospital to check on Hyeon-a and asks Won-jin to help her get inside. The doctors take turns looking in on Hyeon-a while Seong-jin treats the fire victims.

Hee-sook’s Counterstrike Saves the Day

Later, Director Bu, Dr Do, Dr Bae, Su-jeong, Won-jin, Tae-kyeong and Seong-jin are summoned to meet the Magnolia trio, with Sun-ae also in the room. The board decides to suspend the doctors. Mr Gu accuses Tae-kyeong and Won-jin of planning the surgery with Su-jeong ahead of time. The suspensions are announced one by one: Director Bu is permanently relieved of his duties and Su-jeong’s contract is terminated. The board warns of harsher consequences if Hyeon-a doesn’t recover well.

Just as they’re about to leave, Hee-sook walks in with Hyeon-a’s mother. The mother threatens to sue GUMC and the Guseo Foundation if the disciplinary action goes ahead, saying she’ll go after the foundation for hiring a doctor like Su-jeong, who performed a dangerous surgery without IRB approval. It turns out Hee-sook talked her into making that threat to protect the doctors. The Magnolia representatives panic and put the action on hold.

Afterward, Su-jeong thanks Hyeon-a’s mother for helping. The mother thanks her right back, grateful for what Su-jeong did for her daughter.

Posters, a Famous Patient, and an Epilogue That Changes Everything

That night, Tae-kyeong, Seong-jin and Won-jin are making rounds when they spot posters of Gu Yeo-rim, founding director of Guseo Medical Centre, and Kim Seung-hyo, founder of the medical volunteer group White Wings. Tae-kyeong and Won-jin look rattled.

Dr Bae mentions that Seung-hyo is coming to GUMC from the USA for a heart transplant. Hee-sook hears about the transfer too and is shocked. The episode ends with Director Bu learning that Seung-hyo is insisting on surgery at GUMC because his guardian, Won-jin, and his primary surgeon, Su-jeong, are both there.

Then comes the epilogue. Tae-kyeong tries to smother Seung-hyo with a pillow while Won-jin watches. Su-jeong tries to stop him, and Seung-hyo grabs her hand and calls her Doctor X.

Doctor X: Mafia in White Episode 10 Review: A Team Finally Forms, and a Twist Pulls the Floor Out

The opening stretch was a lot of fun. After all that fear and finger-pointing, watching the doctors come to their senses and put patients ahead of a committee that loses nothing if a patient dies felt genuinely satisfying. That said, it stung that Hyeon-a’s mother had to threaten a lawsuit to stop the suspensions. I’m relieved the surgery worked, given how complicated it was, but I’m still worried about Hyeon-a, since she hasn’t woken up yet.

The ending left me confused in the best way. It looks like Tae-kyeong wants to kill Seung-hyo, and my guess is the old man has something to do with whatever happened to Tae-kyeong’s biological parents. It’s also possible Won-jin knows what Seung-hyo did, which would explain why she watches without stopping Tae-kyeong. The Lagota flashback also shows Su-jeong operating on Seung-hyo, so this new transplant would be his second. With only two episodes left, I’m very curious where all of this is heading. These are my theories, and the episode doesn’t confirm any of them.

What Works

  • The OR finally fills up. After an episode of doctors refusing to raise their hands, seeing Director Bu, Dr Bae, Dr Do and Seong-jin show up gives the whole story real payoff.
  • The hand on Hyeon-a’s heart. Su-jeong guiding Hee-sook’s hand is a small, quiet gesture, and it’s the emotional peak of the episode.
  • Seong-gyu’s CCTV argument. It’s a smart way to make the Magnolia Society back down without a shouting match.
  • The bus stop conversation. Hee-sook and Tae-kyeong bonding over families you choose gives the show warmth right before the dark turn.
  • Hee-sook’s counterstrike. Using Hyeon-a’s mother’s lawsuit threat against the board is clever, and the mother’s thank-you to Su-jeong lands.

What Doesn’t Quite Work

  • The rescue leans on a lawsuit threat. It works dramatically, but it’s a bit sad that the way out came from a patient’s parent, not from anyone inside GUMC.
  • Hyeon-a’s recovery is unresolved. She hasn’t woken up, so the victory feels a little unfinished.
  • The Seung-hyo reveal comes fast. The posters, the transfer news and the pillow scene all land within a few minutes, and I wanted more room to breathe before the epilogue.
  • A crowded home stretch. The strike, the suspensions, the Magnolia Society, Tae-kyeong’s past and Seung-hyo’s surgery all need answers in two episodes.

Final Verdict and Rating

Episode 10 gives us the team-up we’d been waiting for, a clever fightback against the board and a closing scene that makes me want to rewatch the whole season for clues. A slightly rushed twist and some loose ends keep it just shy of perfect.

Rating: 9/10
