Doctor X: Mafia in White Episode 5 Recap & Review: Su-jeong's Bluff Breaks Dae-il

Spoiler alert: this recap covers every major event in Episode 5, including the ending.

Doctor X: Mafia in White Episode 5 turns a disturbing discovery into a game of psychological chess. In this SBS medical drama, which airs on Fridays and Saturdays and streams on Netflix, Kim Ji-won’s Gye Su-jeong learns that surgeon Na Dae-il (Jang Seung-jo) left his initials on a patient’s liver. What she does with that knowledge, and how far she’s willing to bluff, makes this one of the sharpest episodes of the series so far.

Episode 5 Recap: A Secret, a Photo, and a Very Expensive Lie

The Initials on Chung-hyeon’s Liver

The episode opens with Su-jeong asking Tae-kyeong to photograph the initials Dae-il carved into Chung-hyeon’s liver. Once the surgery wraps up, Won-jin asks both of them to speak with the patient’s family, but Su-jeong tries hard to wriggle out of it. Chung-hyeon’s wife and daughter thank the team for saving his life, and a comment from the little girl clearly touches Su-jeong. Meanwhile, Tae-kyeong tells Won-jin that Chung-hyeon hasn’t had any surgery since Dae-il operated on him, and he tries to make sense of the inscription.

Tae-kyeong wants Su-jeong to report the whole thing to the hospital. Won-jin pushes back. In her view, telling Chung-hyeon will wreck whatever time he has left, since he’s blissfully unaware right now and nobody can un-tell a truth once it’s out. Su-jeong sees it differently: she says she owes the hospital nothing, but she refuses to hide something from a patient when it’s about his own body.

A “Company Dinner,” a Blackmail Idea, and a Nightmare

Su-jeong goes home for what Hee-sook calls a company dinner with her friends. Over at the hospital, Dae-il stops by the ER and quizzes Seong-jin about the surgery, and he’s visibly surprised to hear that Su-jeong hasn’t exposed the inscription. Back at home, Su-jeong finally tells Hee-sook what she found. Tae-kyeong texts her around the same time, begging her to reconsider going to the hospital board, but Hee-sook has a very different plan: she wants the photos so she can squeeze money out of Director Bu. Su-jeong shuts that down immediately and retreats to her room, still stuck on the inscription.

That night she dreams about attacking military men who carve similar marks into little children’s faces. It’s a short moment, but it says a lot about why this particular wound hits her so hard. The next morning Hee-sook wakes her up for work, and at the board meeting the members are busy praising Dae-il for winning over patients during his month at GUMC. The irony is almost painful.

Telling Chung-hyeon the Truth

Su-jeong, Tae-kyeong, and Won-jin sit down with Chung-hyeon and tell him what Dae-il did. He’s startled at first, but he quickly steers the conversation elsewhere and mentions that his wife will soon give birth at the hospital. Su-jeong reads it as a man who doesn’t want to make a scene.

Tae-kyeong feels guilty about keeping quiet, yet he decides to play ignorant and keep working alongside Dae-il. Won-jin can’t bring herself to write the surgery report because of her own guilt, and when she talks to Su-jeong, Su-jeong admits she feels awful about letting Dae-il’s actions slide.

Dae-il Brags, and Su-jeong Hits Send

Dr Bae and Seong-gyu then warn them that Dr Do and Dae-il are on their way to see Chung-hyeon. Two of the doctors rush to the room, worried about how Dae-il will treat the patient. Chung-hyeon can’t look him in the eye, while Dae-il behaves as if nothing happened. Su-jeong blocks him from touching the patient and asks both doctors to leave.

Dae-il follows her out and brings up the inscription himself, practically bragging about it. That’s the last straw. Su-jeong sends the photos of Chung-hyeon’s liver to Hee-sook, which lets Hee-sook expose Dae-il. Su-jeong goes back to her other patients while Dae-il watches from a distance.

That night Chung-hyeon is struggling with the fact that Dae-il, the man he’d seen as his savior for years, lied to him. At dinner, Dr Do happily says how glad he is to have Dae-il on board, which clearly upsets Won-jin. Dr Bae tells her that Dae-il is scheduled for liver surgery based on his latest test results. Later Su-jeong finds Chung-hyeon crying over the inscription, and he tells her he sees it every time he looks in the mirror. While she tries to comfort him, Hee-sook messages her Dae-il’s test results.

Dae-il Wakes Up to a Haunting Message

The next morning Dae-il comes out of surgery and asks to review the operation tape. Seong-gyu explains that getting approval will be tough, because Dae-il is now the patient. Su-jeong visits his room and draws the initials “D.N” on his food, clearly meant to rattle him.

Dae-il then tries to talk to Dr Mun Yeon-jeong, only to find out that Mun didn’t perform his surgery. He arrived drunk before the operation, and Su-jeong took his place. That scares Dae-il badly. He does get to watch the surgery footage, but he panics because he can’t see clearly what Su-jeong was doing.

Scalpel in the ER

Hee-sook and her friends show up at the ER to take Su-jeong home, and that’s exactly when Dae-il appears and charges at her. He accuses Su-jeong of carving an inscription on his liver. She answers that, unlike the one on Chung-hyeon, which never healed, hers will fade within 10 days.

Dae-il lunges, Tae-kyeong steps in, and Su-jeong shoves him aside. Dae-il slashes her arm with a scalpel. When Hee-sook sees Su-jeong bleeding, she goes ballistic and attacks Dae-il for hurting her highest-earning cash cow. Director Bu arrives after hearing about the confrontation. The surgeons argue over who gets to stitch up Su-jeong’s arm, but she picks Tae-kyeong.

Fired, Investigated, and Tricked

The following day the doctors hear that Dae-il has been fired and is under investigation for what he did to Chung-hyeon. Chung-hyeon drops by with drinks for the team and says he didn’t know what Dae-il had done. Tae-kyeong carries Su-jeong’s jacket into the staff room and sets it on her seat, then hurries off to hand her a fresh lab coat with her name embroidered on it. The doctors also learn that Su-jeong will return to GUMC to replace Dae-il.

Director Bu visits Dae-il in prison and wonders aloud how the inscription survived intact for so many years. Dae-il shows no remorse. Director Bu vows to do everything possible to get his medical license revoked so he can’t hurt another patient, and Dae-il answers by threatening to sue GUMC over what Su-jeong did during his surgery.

Back in the operating room, Tae-kyeong starts to wonder whether Su-jeong really did inscribe Dae-il’s liver. Then Dae-il’s lawyer hands over his test results, and he learns the truth: Su-jeong lied about the inscription to provoke him. The episode closes on Dae-il fuming over being outplayed.

My Review: The Villain Finally Gets a Taste of His Own Medicine

I’ll admit it: this was a strange case, and I still can’t help wondering whether doctors would ever actually do something like this. But strange or not, watching Dae-il fall apart at the mere idea of Su-jeong’s initials on his organs was deeply satisfying. He felt zero remorse about doing the same thing to a patient, yet the second he believed it was happening to him, he lost all composure. That contrast is the entire point of the episode, and it lands.

The bluff itself is the smartest move Su-jeong makes here. She never has to prove anything. She draws on his food, lets his imagination run wild, and lets a blurry surgery tape do the rest. Because she told Dae-il it would fade in 10 days, he spends the rest of the episode terrified of something that doesn’t exist, and I loved that.

What Works

  • The hypocrisy payoff. Seeing Dae-il rattled by his own cruelty gives the story a clean, earned moral punch.
  • The honesty debate. Won-jin’s argument that the truth would only make Chung-hyeon’s remaining life miserable is not written as a strawman, which makes Su-jeong’s stance feel like a real choice rather than an easy one.
  • Chung-hyeon’s breakdown. The scene where he tells Su-jeong he sees the inscription every time he looks in the mirror was quiet and heavy, and it gave the stakes a human face.
  • Hee-sook as comic relief. Her fury at the man who hurt her “highest-earning cash cow” is ridiculous in the best way, and it lightens a dark episode without breaking the tone.
  • Su-jeong and Tae-kyeong. It was funny watching him try to shield her from Dae-il while she kept pushing him out of harm’s way. They’re not a couple yet, but they clearly care about each other, and the embroidered lab coat says it all.

What Doesn’t Work

  • The unanswered “how.” Director Bu asks how the inscription stayed intact for so many years, and the episode doesn’t give a real answer. I want one, because the whole story rests on that detail.
  • Chung-hyeon’s final line. After being told the truth, he turns up with drinks and says he didn’t know what Dae-il had done. I found this confusing and couldn’t tell what it was meant to signal.
  • Losing the ER dynamic. Su-jeong returning to GUMC makes sense, but I’m sad about it, because her scenes with Seong-jin in the ER were genuinely fun.
  • The premise stretches belief. This is a me-problem as much as a writing one, but I kept thinking about how unlikely the whole scenario is in real life.

Final Rating: 8.5/10

A tense, darkly funny episode with a villain who gets exactly what’s coming to him. A few loose threads keep it from a higher score, but this is one of the most satisfying hours of Doctor X: Mafia in White yet.
